2009 APPEALS INFORMATION
YEAR END RATINGS & MIXED EXCLUSIVE APPEALS:
There is no deadline or fee for Year End or Mixed Exclusive Appeals, however allow 3-4 weeks for processing.

SELF-RATE APPEALS:
Self-rate Appeals must be finalized prior to the league registration deadline. There is no fee for self-rate appeals, however allow 3-4 weeks for processing.

MEDICAL APPEALS:
There is a $10 fee for all medical appeals which must accompany the appeals form.
Medical Appeals must be accompanied by a completed Attending Physicians form.
